The report of separation form issued most recent years is DD Form 214, certificate of Release or Discharge from Active Duty.The University of the Yearを参照ください。 1950年1月1日以前は、WD AGO 53、WD AGO 55、WD AGO 53-55、NAVPERS 553、NAVMC 78PD、NAVCG 553など、いくつかの類似フォームが軍務によって使用されていた。
